Use the following forms for adding titles to either the English Literature or the Core Textbook list.
Directions:
1. Click on the name of the appropriate form. Use the English Literature Form for Literature. Use the All Basic Books Form for all other books, including English Textbooks.
2. Fill out the form on your computer by typing in the appropriate information in the highlighted text box form fields and by clicking on the drop-down menus to fill in your Site, the Department, and the Rating for each category. (Type over the existing sample text, or click on the information next to Site, Department or Rating to select the correct information.)
3. Print the fully completed form and submit it to your Department Chair. The form is then discussed at the Department Chair meeting where it will be approved or denied. If the title is approved, the form will be signed by the Department Chair and the Principal.
4. After obtaining these signatures, send the approved form to Robin Ballarin, Director of Curriculum, at the District Office.
Note: Only fully completed textbook adoption forms will be accepted. Please review each form carefully prior to submitting it to the Curriculum Office.
