EL Alternate Assessment Participation Checklist
This form is used by the IEP Team to determine if a student with significant disabilities who is designated as an English Learner requires an alternate assessment to the CELDT (California English Language Development Test).
EL SST Supplementary Referral Form
This form is used when a student classified as an English Learner is being referred to the school's Student Study Team.
EL Reclassification Form for Students with Disabilities
The IEP Team uses this form when redesignating a student with disabilities from an English Learner (EL) to Redesignated - Fluent English Proficient (R-FEP).
