School Site Based Transition Programs

Individual Transition Programs are delivered at the student’s school of residence where students, families, school staff, and agencies work together to plan and provide a set of activities and services that are:
 
Designed for students with disabilities who have not received a high school diploma and are within ages 18-22. 
 
Our programs are:

Special Education Career Connection (SECC)

Special Education Career Connection (SECC)

WorkAbility I (WA) and Transition Partnership Project (TPP) are statewide collaborative programs of the California Department of Education, the State Department of Rehabilitation (DOR), and Grossmont Union High School District. 
 

A variety of programs are designed to provide pre-employment education, employment placement, and follow up services for high school students who participate in special education, and to connect students to DOR through the TPP program. 

SECC Services May Include:

  • Assessments (career, learning style, soft skills, etc.)
  • Career education & exploration 
  • Job Portfolio (career-ready documents)
  • Work experience with community business partners (Intern or Paid)
  • Independent living skills & Self Advocacy
  • Work readiness Curriculum  
  • Industry Speakers 
  • Transition Planning - Post Secondary Program Exploration
